The Integrated Community Care Provider (ICCP) initiative marks a new milestone in our journey — bringing coordinated, accessible, and person-centred care closer to where seniors live.

Through ICCP, SASCO Home works with trusted partners across the community to support seniors in staying active, healthy, connected, and independent — right in their neighbourhood.

What is ICCP?

The Integrated Community Care Provider (ICCP) is a network of care partners working together to ensure seniors receive the right care, at the right time, in the right place.

Instead of navigating multiple services on their own, seniors and caregivers can access a coordinated range of community-based support, all connected through ICCP.

This integrated approach supports ageing-in-place — allowing seniors to live well, safely, and confidently within their own community.

Senior Care Centres (SCCS)

SCCs provide structured care in a safe and supervised environment. Services include:

Dementia care

A supportive space for seniors who need closer care during the day

Active Ageing Centres (AACs)

Our AACs provide a vibrant community space where seniors can stay active and socially engaged. Services include:

Befriending and social programmes

AAC Satellite Centres

AAC Satellite Centres are neighbourhood extensions of SASCO Home’s Active Ageing Centres, located within Residents’ Committees (RCs) and Residents’ Network (RN) spaces.


By bringing programmes into the heart of the community, Satellite Centres make it easier for seniors to stay active, connected, and engaged — just a short walk from home.

A familiar, friendly space to connect with others

Home Therapy (HT)

Our Home Therapy services bring professional care directly to seniors’ homes.

Services include:

Physiotherapy

Occupational therapy

Rehabilitation support to improve mobility and independence

Home Personal Care (HPC+)

For seniors who need support at home, HPC+ services provide practical assistance to support daily living.

Services include:

Housekeeping support

Personal care assistance

Caregiver support and guidance
