Social Worker (5-Day Work Week)

Job Purpose

The Social Worker is responsible for delivering direct social work services, including case management and counselling for clients and their families, as well as conducting thorough social and financial assessments. This role involves developing, implementing, and evaluating casework intervention plans and programmes to support clients in adapting to their limitations, while guiding families through important care-related decisions. The jobholder is also expected to uphold the highest standards of professionalism in social work and embody SSCH’s philosophy of person-centred care.

Key Responsibilities

  • Review applications received from the Agency for Integrated Care (AIC), conduct interviews with clients and their caregivers during the enrolment process into the programme or Home, and carry out comprehensive biopsychosocial assessments to evaluate the strengths, resilience, and support systems of clients and their families.
  • Build rapport with residents/clients and their families by maintaining consistent communication, addressing their concerns, and supporting their goals. Encourage a positive outlook in clients and educate family members on the physical and psychosocial needs of their loved ones, while guiding them on how to access both internal and external support resources.
  • Provide casework management for our clients and coordination with external agencies such as hospitals and polyclinic for clients who require financial assistance for their medical appointments.
  • Develop and implement Individual Care Plans (ICP) tailored to each client’s needs, in collaboration with the multidisciplinary team (MDT), and provide therapeutic support as required to enhance clients’ well-being and quality of life.
  • Execute the Individual Care Plan (ICP) and monitor clients’ progress regularly. Develop and implement structured programmes and activities that are aligned with SASCO’s core values, ensuring they effectively address and support the care needs of each client.
  • Coordinate with PT and OT departments to assist in helping clients feel comfortable and confident.
  • Prepare and submit 6-monthly Individual Care Plan (ICP) review reports and maintain monthly review records for all clients, ensuring timely documentation and compliance with care standards.
  • Evaluate existing, new and prospective clients based on their needs, desires and limitations.
  • Update clients’ particulars and social report/review. Conduct casework evaluation, maintain case notes/documents.
  • Provide person-centred counselling to clients and/or their families, focusing on individuals experiencing poor mental health conditions such as depression and anxiety, as well as those facing coping and adjustment challenges during periods of transition.
  • Conduct risk assessments to evaluate the potential for self-harm (e.g. suicide) and harm to others, including cases involving family violence or elder abuse, and take appropriate follow-up actions in accordance with established protocols.
  • Conduct financial assessments and manage funding support for clients, including identifying and referring cases for financial assistance. Responsibilities include generating and reviewing key reports such as the Means Test Subsidy Change Rate Report from MOH (for invoice printing), the Changed Subsidy Report, and the IHDC Master List, as well as preparing periodic financial reports as required for monitoring and compliance purposes.
  • Any other additional tasks assigned by the Management.

Qualifications & Skills Required

  • Degree in Social Work and registered as a Registered Social Worker (RSW).
  • At least 2 years’ social work experience.
  • Prior experience in residential setting for the elderly.
  • Strong knowledge of government financial schemes.
  • Ability to work effectively with diverse groups including seniors, families, and stakeholders.
  • Proficiency in local dialects is an advantage.
  • Excellent written, verbal, and interpersonal communication skills.
  • Ability to generate reports and compile statistical data as required by the Ministry of Social and Family Development (MSF).

Care Planner and Coordinator, Community Care Integration (5-Day Work Week)

Job Purpose

The Care Planner and Coordinator is primarily responsible for translating InterRAI assessments into personalised care plans and coordinate seamless service delivery for seniors. Act as a key link between clients, Care Assessors, and community partners to ensure timely access to medical, social, and community services.

Key Responsibilities

  • Develop person-centred care plans based on InterRAI assessments, client needs, and preferences, detailing goals, interventions, and recommended services.
  • Coordinate referrals and service delivery, monitor client acceptance, and follow up on service commencement and progress.
  • Act as a client touchpoint for queries and support from referral to service commencement.
  • Update service availability, referral status, and alert Care Assessors to client condition changes.
  • Maintain compliance and documentation standards; review and update care plans every 6 months or as needed.
  • Build partnerships and facilitate communication across providers and multidisciplinary teams.

Qualifications & Skills Required

  • Bachelor’s degree in healthcare management, gerontology, or related field.
  • Healthcare or Allied Health professional (nurse, social worker, therapist) OR care staff with >2 years’ experience in assessments, care coordination, or case management, preferably in senior/long-term care.
  • Prior experience in care coordination/case management (e.g., patient navigator, case manager).
  • Understanding of local eldercare services, grants, and sector workflows.
  • Have experience using InterRAI IT, BRIGHT and Community Care Plan systems.

Care Assessor, Community Care Integration (5-Day Work Week)

Job Purpose

The Care Assessor is primarily responsible for supporting the seamless delivery of integrated care for seniors by conducting standardised InterRAI assessments and coordinating with Care Planners, Care Coordinators, and community partners (SCCs, AACs, HPC+, HT) to ensure holistic support.

Key Responsibilities

  • Make first contact with clients (e.g., via home visits) to conduct initial assessments.
  • Administer InterRAI Check-ups for clients in accordance with InterRAI standards, supplemented with accurate data from clients, caregivers, and available documentation (e.g., health records, social reports, electronic systems, and case notes) where relevant.
  • Conduct re-assessments for clients on a 6-monthly basis or as needed.
  • Ensure compliance with documentation standards for InterRAI assessments.

Qualifications & Skills Required

  • Bachelor’s degree in healthcare management, gerontology, or a related field.
  • Healthcare or Allied Health professionals (e.g. nurse, social worker, therapist) OR
  • Care staff with prior experience (> 2 years) in assessments, care coordination and/or case management, preferably in senior or long-term care sector.
  • Have experience using InterRAI IT system and Community Care Plan system.

Social Media Intern (5-Day Work Week)

Job Purpose

We are seeking a creative and driven Social Media & Marketing Intern to support our Branding, Fund & Resource Management team in executing impactful campaigns and producing engaging content across multiple platforms.

This internship provides hands-on experience in a fast-paced, purpose-driven environment, offering opportunities to contribute ideas, develop marketing skills, and help shape the organisation’s brand presence and outreach efforts.

Key Responsibilities 

  • Conduct research to identify market trends, analyse competitor strategies, and understand customer preferences.
  • Develop engaging content tailored for social media platforms, websites, and email marketing campaigns.
  • Support the planning and management of social media activities, including content creation, visual design, audience engagement, and performance analytics.
  • Assist in the end-to-end execution of marketing campaigns, from initial planning through implementation and performance evaluation.
  • Carry out additional tasks as assigned by the Head of Department to support broader marketing and resource initiatives.

Qualifications & Skills Required

  • Currently pursuing a diploma / degree in Digital Design & Media, Marketing or a related field.
  • Strong interest in digital marketing and social media trends.
  • Excellent communication and interpersonal skills.
  • Creative mindset with a flair for storytelling and design.
